MEDICARE
Medicare patients will be eligible to receive a maximum of one manual manipulation per member per day and a maximum of up to 30 manual manipulations per member per calendar year. All visits are subject to deductible, co-insurance and other policy limitations. Additional visits during 2019 would be considered non-covered and will be your financial responsibility.
BLUE CROSS
Chiropractic visits are limited to up to 20 per calendar year subject to medical necessity. All visits are subject to deductible, co-insurance and other policy limitations. Additional visits during 2019 would be considered non-covered by Blue Cross and will have a $36.00 per visit patient financial responsibility.
DELAWARE MEDICAID
Chiropractic benefits through Delaware Medicaid are available to Highmark Healthy Options insured only. Chiropractic benefits are limited to up to 24 visits/year subject to medical necessity and policy limitations. Additional visits during 2019 would be considered non-covered by Delaware Medicaid and will be your financial responsibility.
OTHER INSURANCE PLANS
Chiropractic benefits must be verified before starting treatment. A patient responsibility of $36.00 per visit will be assessed until benefits verified. Policy limitations may effect final financial responsibility.
NON-INSURED PATIENTS
For patients that either have no insurance coverage or choose not to use their insurance plans for treatment in our office we offer the following non-insured rates:
New patient initial examination and adjustment $45.00
Routine examination and adjustment $40.00
Tuesday and Saturday discount adjustment day $30.00
